서론

화장품은 ‘인체를 청결·미화하여 매력을 더하고 용모를 밝게 변화시키거나 피부・모발의 건강을 유지 또는 증진하기 위하여 인체에 바르고 문지르거나 뿌리는 등 이와 유사한 방법으로 사용되는 물품으로서 인체에 대한 작용이 경미한 것’이라고 화장품법에 정의되어 있다.[1] 피부에 대한 화장품의 효능을 평가하기 위해 다양한 측정기술들이 개발되고 있으며, 임상 평가 기관에서는 개발된 측정기기를 이용하여 많은 화장품의 효능을 평가하고 있다. 본 리뷰에서는 최근 개발된 측정기기 중에서 in vivo reflectance confocal microscope에 대해 보고되었던 논문을 중심으로 화장품 효능평가에의 응용 가능성을 살펴보고자 한다.


피부의 다양한 효능 평가를 위한 방법들

화장품은 피부의 건강과 아름다움에 도움을 주면서, 피부에 대한 효능을 제시하고 있다. 대부분의 효능이 소비자가 평소에 느끼고 체감하는 것으로, 쉽게 이해하며 인지하는 것들인데, 화장품 회사의 너무 과장된 표현을 금지하기 위해 식품의약품안전처에서는 화장품 표시 및 광고 범위를 관리하기 위한 가이드라인을 제정하였고,[1,2] 이와 같은 표현의 표시 또는 광고가 가능할 수 있도록 화장품에 대한 인체적용시험이 실시되고 있다.

화장품의 효능에 대한 평가는 대부분 비침습적 방법(non-invasive method)을 이용하여 평가되고 있다. 대부분의 효능이 화장품 표시광고 표현 범위에 해당되며, 주로 보습, 장벽기능, 탄력, 피부색, 피부결, 주름 등이 포함된다. 피부는 크게 표피층과 진피층으로 구분할 수 있으며, 표피층은 각질층, 과립층, 유극층, 기저층으로 다시 구분된다. 여러종류의 세포와 여러 가지 기관으로 구성된 피부의 기능은 매우 복잡하다. Jorgen Serup등은 그동안 개발되고 연구되어 온 비침습적 방법들을 정리하여 ‘Handbook of non-invasive methods and the skin’이라는 책을 발간하였다.[3] 다음의 표 1과 2는 이 책의 내용을 중심으로 피부 구조학적 및 기능적 관점에서 어떤 평가들이 가능한 것인가에 대해 간략하게 정리 요약한 것이다.

표피는 피부의 가장 바깥쪽에 위치하는 구조로, 피부 표면 특성의 확대 분석을 통해 피부 건조, 미세피부결, 피부 주름 등을 평가할 수 있다. 또한 표피의 기능적 특성을 통해 각질박리 속도, scale을 통한 피부 건조상태 평가, 피부 보습, 피부 장벽기능, 피부표면 pH 평가등이 가능하다. 진피는 피부의 형태를 유지시켜 주는 지지층으로 주로 결합조직 섬유의 생성과 분해 조절에 관여하고, 신경과 혈관이 분포하고 있다. 진피의 구조와 기능의 이해를 통해 ultrasound 기술을 이용하여 진피층 결합조직의 치밀함을 평가할 수 있고, 피부 탄력과 관련된 평가를 할 수 있다. 그리고 laser doppler 기술을 이용하여 피부내 모세혈관 혈행을 평가할 수 있고, 여기에 imaging 기술이 융합되어 모세혈과 혈행에 대한 이미지 분석이 가능하다. 피지분비량 평가는 피지흡착성이 높은 sebu-tape를 이용하여 평가가 가능하고, 피지 분비 속도는 gravimetric 기술을 적용한 기기를 활용하여 평가할 수 있다.


최신 연구 동향

진단 및 분석 기술의 발달로 단순 특정 기능만을 평가하는 한계를 넘어서 피부의 기능을 복합적으로 평가할 수 있는 가능성이 높아지고 있다. 해상도가 높고 실시간으로(real-time) 측정이 가능하며 비침습적으로 피부내부의 변화를 측정할 수 있는 기기가 개발되고 있으며, OCT(optical coherence tomography), in vivo 공초점 현미경(in vivo confocal laser scanning microscope) 등의 기기의 발달로 피부 내부의 변화를 정성적으로 분석할 수 있는 가능성이 높아졌다. 좀 더 해상력을 높인 고조파 발생 현미경(harmonic generation microscope)과 다광자 현미경(multiphoton microscope) 등의 기술이 개발되어 피부 세포의 크기뿐만 아니라 피부세포의 핵 크기까지 분석할 수 있게 되었으며, 피부 콜라겐 변화까지도 측정이 가능하다고 한다. 지금까지 피부 표면의 변화와 단편적 피부 기능 측정만을 이용한 부분적 효능평가였다면, 이제는 피부 속 변화까지 고려한 종합적 피부 기능 평가가 가능한 시대가 도래했다고 할 수 있다. 또한 피부 각 기능의 메커니즘 연구에도 많은 도움이 될 것이라고 생각된다.

일본의 다카하시 모토지(M.Takahashi)는 피부상태나 유용성 평가에 사용되고 있는 피부 평가법을 다음의 표 3과 같이 1세대, 2세대, 그리고 3세대 측정기술로 분류하였다.[4] 1세대 측정 기기는 대부분 피부 표면에서의 특성 변화를 측정하는 기기이며, 2세대, 3세대 측정기기로 갈수록 피부 내부의 구조와 구성 성분을 높은 분해능으로 측정할 수 기기가 개발 연구되고 있다. 여기에서는 in vivo reflectance confocal microscope(RCM)을 이용한 연구내용을 중심으로 화장품에의 응용 가능성을 살펴보고자 한다. in vivo confocal laser scanning microscope(CLSM)으로 불리기도 하지만, 본 내용에서는 용어를R CM으로 통일해 표기한다.

in vivo reflectance confocal microscope(RCM)와 피부

일반적으로 피부 속 변화를 관찰하기 위해선 생검(biopsy) 샘플에 대한 여러 과정(fixation, sectioning, mounting, staining)을 거치게 되고, 이러한 과정은 피부를 구성하고 있는 세포나 구성물의 변형을 일으킬 수 있어서 정확한 실제 상태를 반영하는 것이라고 하기 어렵다. [5] 하지만, RCM이 개발되면서 동일한 부위(same site)에 대하여 실시간(real-time)으로 비침습적으로 피부 내부의 측정이 가능하게 되었고, H&E histology staining 과의 비교 연구 결과가 보고되면서, 피부 내부의 구조와 구성 성분의 변화를 histology 과정 없이 측정할 수 있음을 알게 되었다. 그림 1과 그림 2는 H&E staining과 RCM 이미지와의 높은 상관성을 보여주고 있다.[6, 7] 또한 그림 3은 RCM을 이용하여 여러 부위의 피부 내부를 비교 분석한 것으로 햇빛에 노출된 부위(sun-exposed site)와 햇빛에 적게 노출된 부위(sun-protected site)간의 특성비교가 가능함을 보여주고 있다.[7] granular keratinocyte의 수는 햇빛에 노출된 부위의 피부에서 높고, spinous keratinocyte의 수는 반대로 햇빛에 적게 노출된 부위의 피부에서 높은 현상을 보여주고 있다. 피부 혈관의 수는 햇빛에 노출된 부위의 피부에서 높은 결과를 보여주고 있다.

Parameters for RCM study

RCM은 피부에 어떤 외과적 시술 없이도 피부 내부에 대한 이미지를 현미경처럼 해상력 높게 얻을 수 있는 측정 기기이다. 깊이별로 피부의 이미지를 얻을 수 있고, 또한 이 이미지들을 모아서 입체적으로 분석할 수도 있다. C.Longo 등[8]은 RCM을 이용한 parameter들을 피부 깊이별로 구분하여 연구를 진행하였고, 피부두께(epidermal thickness), 피부결(furrow aspects), 표피 세포들의 상태에 대해서는 honeycomb pattern과 mottled pigmentation으로 분류하였다. DEJ(dermo-epidermal junction) 부위는 polycyclic papillary contours를 이용하였고, 진피층 콜라겐의 경우 thin reticulated collagen, coarse collagen, huddle collagen, curled bright structure의 4가지 형태로 구분하였으며, 그 내용은 표 4와 같다.

연령대를 기준으로 5 그룹으로 나누고(35세 이하, 36-45세, 45-55세, 56-65세, 65세이상), 각 그룹당 10명에 해당하는 피험자의 피부를 통해 분석한 결과는 그림 4와 같으며, RCM을 통해 표피와 진피 특성을 측정할 수 있음을 확인할 수 있다.

RCM을 이용한 연구에 이용되는 parameter들은 비슷하지만, 참고를 위해 다음 표 5를 소개한다.[9] 이러한 parameter들을 활용한다면 화장품 평가에 유용할 것으로 생각된다.

표 5의 parameter들을 이용하여 120명의 volunteers에 대한 결과는 표 6과 같으며, aging에 따라 furrow depth, irregular honeycomb pattern, mottled pigmentation, polycyclic papillary contours, curled bright structures, huddled collagen 등이 증가하고 있고, number of papillae, thin reticulated collagen 등은 연령에 따라 감소하고 있다. 이러한 사실을 통해 연령에 따른 피부에서의 RCM parameter들의 비교가 가능함을 알 수 있다.[9]

보습제품의 피부에 미치는 변화 연구

10명의 volunteers를 대상으로 진행된 시험으로, RCM을 이용하여 furrow’s size, overall interkeratinocyte reflectance, furrow’s morphology, skin surface irregularity 등의 parameter를 측정하였고, 보습제품 사용 전후에 따라 RCM parameter 들의 값이 변화하고 있음을 보고하였다.[10] 그림 5는 보습제품 사용전후에 따라 furrow’s size가 감소하는 결과이고, 그림 6은 skin surface irregularity가 제품 사용에 따라 감소하는 것을 보여주고 있으며, 그림 7은 보습제품 사용에 따라 keratinocyte 경계면이 좀더 밝아지고 명확해지고 있음을 보여주고 있다. RCM이 보습제품에 의한 피부 표면 및 내부의 변화를 연구하는데 유용한 수단임을 시사해 주고 있다.

노화에 따른 dermal papilla structure 변화 연구

일본 여성 90명을 대상으로 노화에 따른 dermal papilla 구조의 변화를 RCM을 이용하여 dermal papilla의 개수와 형태를 분석한 결과, 노화에 따라 dermal papilla 개수가 감소함이 보고되었다.[11] 특히 dermal papilla 구조를 개수, 높이(height), 그리고 면적(cross-sectional area of dermal papilla) 으로 구분하여 분석한 결과, 개수와 높이는 통계적으로 유의하게 감소하였고, 면적은 반대로 통계적으로 유의하게 증가한 결과를 보였다. 여기서 주목할 내용은 dermal papilla의 구조를 세분화하여 분석한 방법이며, dermal papilla의 윗부분(top)과 아랫부분(bottom)을 4 부분으로 구분한 것으로 그 내용은 그림 8과 같다. 이러한 방법을 이용한 다면 anti-aging 연구에 많은 도움이 될 것으로 생각된다

민감피부 평가 연구

다음은 RCM을 이용하여 민감피부의 평가에 이용한 연구의 예이다. 166명을 대상으로 설문평가와 lactic acid sting test를 통해 구분된 민감성 피부 그룹과 healthy control 그룹의 피부를 RCM을 이용하여 honeycomb pattern, spongiform edema, dermal ring pattern, epidermal thickness의 parameter를 측정하고, 민감성 피부 그룹과의 상관관계를 확인한 결과, disarranged honeycomb pattern과 spongiform edema에서 민감성 피부 그룹과 healthy control 그룹 사이에 통계적으로 유의한 차이를 보여주었다.[12] 화장품의 유효성 평가와는 달리 민감성 피부의 평가가 쉽지 않음을 고려할 때 민감성 피부와 상관관계가 높은 RCM parameter를 이용한다면 민감성 피부 판정에 유용한 방법이 될 것으로 사료된다.


여드름 평가 연구

M.Manfredini 등[13]은 RCM을 이용하여 여드름 피부의 infundibular size, border of infundibulum, contents within infundibulum, exfoliating keratinocyte, exocytosis, dermal inflammation 등의 parameter(표 7)를 통해 여드름 피부와 정상피부의 비교평가를 연구하였고, 그 결과 RCM parameter 들의 정량적 비교가 가능함을 확인하였다. 또한 여드름 경험이 있는 피부와 여드름 경험이 없는 피부에서는 hair follicle 주변의 bright thin contour가 큰 차별성을 보여주는 특징임을 확인하였다(그림 9). 여드름 평가의 육안평가와 병행하여 피부 내부의 변화의 평가를 진행한다면 여드름성 피부 완화를 위한 화장품 개발에 많은 도움을 줄 것으로 생각된다.

결론

이 외에도 RCM을 이용한 연구는 다방면에서 활발하게 진행되고 있다. 여기에서 소개하지는 않았지만, skin surface cellular morphology 분석을[14] 포함하여 여러 분야에서 연구가 활발한 편이다. 해상력과 분해능력이 뛰어나고, 실시간으로 동일한 부위의 전후 비교를 할 수 있으며, 외과적 시술이 필요 없는 비침습적(non-invasive) 측정기기가 연구 개발되면서, 그 응용가능성도 점점 넓어지고 있다. 경제적인 측면에서 쉽게 접근할 수 없는 고가의 장비라는 단점이 있지만, 그 동안 접근할 수 없었던 피부 내부의 구조와 구성성분의 변화를 측정할 수 있다는 장점은 매우 고무적이다. 정성적 평가의 한계를 넘어서 이젠 정량화도 가능할 수 있을 정도로 분석기능이 발달하고 있다. 이에 따라 그동안 화장품 적용에 따른 마지막 결과에 해당하는 피부의 효과를 평가했던 부분과 화장품 적용에 의한 변화하는 피부내부의 과정을 측정할 수 있는 비침습적 기기를 병행한다면, 보다 정확한 화장품 효능 평가는 물론 좀 더 효과적인 유효성분 개발도 가능할 것이라 생각된다
