Hypoxia Improves Hair Inductivity of Dermal Papila Cells via Nuclear NADPH Oxidase 4-Mediated Reactive Oxygen Species Generation

[더케이뷰티사이언스]   탈모 환자들을 위한 새로운 세포치료제가 개발되었다. 이 연구 내용은 지난 1월 31일 '영국피부학회지(British Journal of Dermatology)'에 게재되었으며, 미래창조과학부의 '중견연구자지원사업' 일환이다.

 피부 질환에 대한 세포 치료제 개발은 체세포 치료제와 줄기세포 치료제 형태로 개발되고 있는데, 모유두 세포는 모발 성장에 중요한 세포임에도 불구하고 여러 한계점 때문에 탈모치료제로는 개발이 어려웠다. 그 한계점을 극복하고 탈모의 세포치료제를 직접 개발한 스템모어 대표이자 연세대학교 성종혁 교수 연구팀에게 이야기를 들어보았다. 성종혁 교수가 대표로 있는 스템모어는 연세기술지주회사의 자회사로 2015년 설립되었으며, 탈모치료제 연구개발을 주력으로 하고 있는 회사이다.

 

모유두 세포를 선택하여 탈모 세포치료제를 만들게 된 연구배경이 궁금합니다.

스템모어 연구소에서는 지방줄기세포 및 분비단백질을 연구하고, 피부 및 모발 재생에 적용하는 일을 진행해왔습니다. 그렇지만 다량의 세포를 쉽게 얻을 수 있고 탈모를 치료하는 효과가 있음에도 불구하고, 지방줄기세포의 효과는 경쟁제품에 비해 크지 않았고 새로운 모낭을 형성하지 못했습니다. 이를 대체할 수 있는 세포를 찾게 되었고, 모유두 세포를 연구하게 되었습니다.

 

연구 내용에 대해 소개해주세요.

탈모의 세포치료를 위해 가장 적합한 세포 원료는 모유두 세포입니다. 이 모유두 세포는 모발 발생 및 성장에 있어서 굉장히 중요한 세포이기 때문입니다. 실제로, 두피로부터 이 모유두 세포를 채집해서 현적배양 후 표피세포와 혼합하여 누드마우스에 이식했을 때 모발이 새로 자라났습니다. 하지만 이 세포를 탈모치료에 사용하기에는 몇 가지 극복하기 어려운 점이 있습니다. 1)이 세포를 두피로부터 분리해내는 점 2)세포의 수를 증식시키는 점 3)증식시키기 위해서 6~7번 계대 배양해야 하는데, 이 과정에서 모발 발생 및 성장 능력이 현저하게 감소해버린다는 문제가 있습니다. 이러한 한계점이 모유두 세포가 탈모의 세포치료를 위해 가장 적합함에도 불구하고 아직까지 제대로 개발되고 있지 못하는 이유입니다.

또한, 저산소 환경은 세포 증식, 혈관신생 등의 다양한 생리학적 프로세스를 조절하는 것으로 알려져 있습니다. 앞서 언급한 모유두 세포의 한계적 성질 때문에 탈모 치료와 관련하여 많은 수의 세포가 필요한 연구나 세포 자체를 활용한 치료에 사용하는 데 큰 어려움이 있었습니다. 그래서 저희 연구진은 이 세포의 증식을 증가시키는 동시에 그 기능(Mainly hair gross)을 유지시킬 수 있는 저산소(Hypoxia) 배양 이라는 특별한 배양조건을 활용하였습니다. 저산소 조건은 모유두 세포의 증식을 증가시키는 동시에 세포 노화를 지연시킨다는 것을 밝혀내었고, 이에 그치지 않고 모유두 세포의 기능 또한 보전되어 Hair inductivity를 촉진한다는 것을 동물실험을 통해 확인하였습니다. 즉, 저산소 조건배양법을 활용하여 합성화합물 전처리, 유전자 재조합 등에 비해 안전하고 새로운 배양법을 개발함으로써 모유두 세포의 기존 한계를 돌파하고, 극복했다고 할 수 있습니다.

주요 실험 결과에 대해 설명해주세요.

기존 모유두 세포 배양법을 사용하면 6~7 계대 배양 후 모낭 생성능력이 사라져 버립니다. 우리 연구팀은 다양한 농도의 산소조건을 이용하여 배양하는 실험을 수행하였고, 모유두 세포를 저산소 조건(2% 산소 조건)에서 배양하였을 때, 모유두 세포의 증식력이 획기적으로 개선되었고 저산소 조건에서 8~9계대까지 배양한 모유두 세포를 이식한 경우에도 새로 모낭이 형성됨을 패치 어세이(Patch assay)를 통해 확인하였습니다. 또한 저산소 조건에서 배양한 모유두 세포를 생쥐에 이식하면 생존력이 증가하고, 모낭의 외측 모근초 세포(ORS: out-root-sheath)의 증식을 획기적으로 높인다는 결과를 얻었습니다.

실험을 진행하면서 어려웠던 점이 있었다면?

실험에 충분한 모유두 세포를 확보하는 것이었습니다. 모낭으로부터 분리하는 기간도 오래 걸리고, 실험에 필요한 세포를 확보하는 것이 가장 어려웠습니다. 결국 여러 환자들의 두피를 얻어서 실험을 진행하게 되었습니다. (더케이뷰티사이언스를 통해) 모낭을 공급해주신 공여자 및 피부과 교수님께 깊이 감사드립니다.

 

활성 산소로 인해 모유두 세포의 증식 및 성장인자 발현 증가를 확인하신 메커니즘에 대해 소개해주세요.

저산소배양시 활성산소가 신호전달물질로 작용하여, 모유두 세포의 증식 및 성장인자 발현을 높이는 것을 확인하였습니다. 모유두 세포에서 NOX4 유전자가 핵 부위에 존재하고, 저산소배양은 NOX4를 통해서 활성산소의 생성을 높인다는 것입니다. 또한 안티마이신(Antimycin) 및 로테논(Rotenone) 등 활성산소를 발생하는 물질의 처리에 의해서도 모유두 세포 증식이 증가함을 확인하였고, 동물실험을 통해 활성산소 전처리한 모유두 세포가 모낭형성이 증가함을 확인하였습니다.

연구 결과 후 임상시험은 2020년부터 시작되는데, 현재 진행 중인 추가 연구가 있다면?

현재 비임상 독성시험을 진행하고 있으며, 추가로 임상시험용 세포를 생산할 수 있는 GMP 시설을 갖추고 있습니다. 올해 말 식약처의 허가를 받으면, 내년부터 환자들에게 임상시험을 진행할 예정입니다.

 

이번 연구 결과로 인해 기대되는 성과는 무엇인가요?

그동안 치료에 충분한 세포의 확보가 어려워서 개발이 어려웠던 모유두 세포의 증식을 획기적으로 개선하였고, 이는 합성화합물 전처리 등에 비해 안전한 새로운 배양법입니다. 현재 적용되고 있는 모발이식은 뒷덜미 모발을 정수리부분에 이식하는 시술로서, 이식할 수 있는 모발 수와 가능한 시술 횟수(1~2회)에 한계점을 갖고 있습니다. 더불어 너무 무리하게 많은 수의 모발을 이식함에 따라 나타나는 부작용도 보고되고 있습니다.

이에 반해 본 기술을 적용하면 부작용도 적고, 환자가 원할 때 배양된 모유두 세포를 여러 번 시술할 수 있는 장점이 있습니다. 모유두 세포를 이용한 세포치료제에 대한 임상시험을 예정 중이고, 시판 허가 후에는 모발 이식을 대체할 수 있을 것으로 기대됩니다.

 

앞으로의 연구 계획이 궁금합니다.

이번 연구를 통해 저희 연구진은 저산소 조건에서 배양한 모유두 세포의 발모촉진 효과를 확인하였고, 세포치료제로 개발 가능성이 있음을 확인했습니다. 임상시험을 통해 환자에게 실제로 효과가 있는지 확인하는 연구를 진행하여 기존의 한계점을 가진 이식수술을 대체할 수 있는 치료제 연구를 지속해서 해 나갈 예정입니다.

또한, 최근에 지방줄기세포를 모유두 세포로 전환하는 기술도 추가적으로 개발하였습니다. 헤어 주기를 조절하는 신규 단백질도 발견함에 따라 이런 기술들을 이용해 탈모를 치료할 수 있는 의약품을 개발하고 상용화하는 것이 목표입니다.


SUMMARY

BACKGROUND

Dermal papilla cells (DPCs) play a key role in hair regeneration and morphogenesis. Therefore, tremendous efforts have been made to promote DPCsʼ hair inductivity.

OBJECTIVES

The aim of this study was to investigate the mitogenic and hair inductive effects of hypoxia on DPCs and examine the underlying mechanism of hypoxia-induced stimulation of DPCs.

RESULTS

Hypoxia significantly increased the proliferation and delayed the senescence of DPCs via Akt phosphorylation and downstream pathways. Hypoxia upregulated the growth factor secretion of DPCs through mitogen-activated protein kinase pathway. Hypoxia-preconditioned DPCs induced the telogen-to-anagen transition in C3H mice, and also enhanced hair neogenesis in a hair reconstitution assay. Injected GFP-labeled DPCs migrated to outer-root sheath of hair follicle, and hypoxia-preconditioning increased the survival and migration of DPCs in vivo. Conditioned medium obtained from hypoxia increased the hair length of mouse vibrissa follicles via upregulation of alkaline phosphatase, vascular endothelial growth factor, and glial cell line-derived neurotrophic factor. We examined the mechanism of this hypoxia-induced stimulation, and found that reactive oxygen species (ROS) play a key role. For example, inhibiting of ROS generation by N-acetyl-L-cysteine or diphenyleneiodonium treatment attenuated DPCsʼ hypoxia-induced stimulation, but treatment with ROS donors induced mitogenic effects and anagen transition. NADPH oxidase 4 (NOX4) is highly expressed in DPCsʼ nuclear region, and NOX4 knockout by CRISPR-Cas9 attenuated the hypoxia-induced stimulation of DPCs.

CONCLUSIONS

Our results suggest that DPC culture under hypoxia has great advantages over normoxia, and is a novel solution for producing DPCs for cell therapy.
